Required Documents for Registration:
- Birth Certificate
- Current Valid Shot Record on the Florida 680 Form
- Current Physical completed within the past 365 days
- Valid proof of residence [IE: Lease if you rent, Warranty Deed or Homestead Exemption if you own] AND a copy of your most recent electric bill
- Verification of Residence from Student Enrollment, if you are living with someone (See above link for information)
- Copy of Parent's Drivers License.
